Forms you will have to bring with you or have to digitally submit:
-If you hire for the first time or haven't hired for longer then 3 months from us, you will have to fill out the Millens Hire Questionnaire Long Form (see below) and get the Driving Licence Check Code (see below). Also bring the documents mentioned at the end of the Millens Hire Questionnaire Long Form.

-If you have hired from us not longer then 3 months ago, you can fill out the Millens Hire Questionnaire Short Form (see below) and the Driving Licence Check Code (see below) if the last hire was longer then 21 days ago.
